- First, enter the tower south-east of Zarrith. It is south of the main mountain of Sheol and has a level 200 requirement.
- Enter the teleport. Up to 4 players can enter. Players who have completed the quest previously may enter again although cannot spawn the creatures. There must be at least one player who has not yet completed the quest.
- Have the player/s who have not yet finished the quest "use" the blood fountains inside. Each time the fountain is used a wave of creatures will spawn. These creatures can include the following: Nightmare, Black Widow, Deranger, and Bedsore. There can be multiples of each. Each wave cleared will lower Delora Sanguis' health. You cannot attack her during this phase.
- At some point during the fight (seems to be random?) the boss will teleport to the northern side of the room and fight you. From now on, waves will spawn independently of your actions. They appear to have about a 50% chance to spawn every 10 second interval.
- When the boss is dead, you will be teleported to the reward room after 30 seconds. You can choose one of the rewards.
